Mercy Health Partners' High School Student Heart Screenings
MHP Student Heart Screenings are free and
open to any High School student .

The heart screenings attempt to identify any pre-existing heart conditions that could potentially increase a student's risk of vigorous physical activity and/or athletic competition. High School Student Heart Screenings are a five-step process that take place at Mercy Health Partners' Hackley Campus Youngberg Auditorium and are completed in approximately 15 minutes:
Station 1: Check in
Station 2: Healthy Heart Check History Questionnaire review and blood pressure check
Station 3: 12-lead EKG
Station 4: Echocardiogram (if indicated)
Station 5: Check out
Your child's records will be reviewed by a cardiologist within a few days. Within 10 days, you will receive either a letter indicating your child is at low risk or a request for a follow up visit with a physician.

To Register for MHP's High School Student Heart Screenings:
MHP Student Heart Screenings are free and open to any High School student .
Each student must have a parent or guardian's consent to undergo the screening. The student athlete must be in 9th, 10th, 11th, or 12th grade and born between January 1, 1993 and December 31, 1998.

Steps to complete the online registration:
1. Print the consent form and get parent or guardian's signature
2. Print the healthy heart check history questionnaire, complete it and have a parent or guardian sign it.
3. Register for one of the open appointment times
4. Print the appointment and keep as your appointment reminder
**Important!**
If your student is attending a 2012 or 2013 screening, it is NOT necessary to bring a copy of the student's sports physical form to the heart screening. Students must bring both the signed consent and signed healthy heart check history questionnaire to the appointment. Parents are welcome to accompany their student athlete.

Screenings take approximately 10 to 30 minutes to complete.
